Referrals:

 
Provider/Specialty Offices:
  • Please fax Referral requests, copies of consultation notes, and/or appointment updates to 405-644-8237
  • Requests will be processed as soon as possible but no later than 30 days from receipt
Patients:
  • Routine Referral requests are normally processed and sent to specialty offices within 7 workdays of receipt from your clinician
  • You will receive a text from Variety Care with the Referral Specialty Office’s name and phone number the day it is processed
  • When texting is not an option for you as a patient, you will receive a call from Variety Care to provide Referral details, voicemail for call back if no answer and/or letter mailed to provide Specialty Office name and number
  • After being processed, specialty offices will review requests and documents and contact you to schedule within 1 to 2 weeks. In the event you do not hear from the Specialty Office within 1-2 weeks, it is recommended to contact them directly to speed up the process
  • You can view your referral information in your Patient Portal under the Referrals section
  • To follow up on referrals, you may fill out this form.
