Leaving a Compliment or Grievance
As a HRSA grantee, Variety Care abides by federal, state, county and local policies with regard to race, color, or national origin in accordance with Title VI of the Civil Rights Act of 1964. Any person who believes they have been aggrieved by any unlawful discriminatory practice under Title VI may file a complaint with Variety Care through the reporting feature on the virtual contact us grievance portal, by contacting the Variety Care Compliance and Privacy Officer by phone at 405-632-6688 or by submitting a written compliant to the Compliance and Privacy Officer at 3000 N. Grand Blvd, Oklahoma City, OK 73107.
All Variety Care patients have the right to file a complaint with Variety Care. It is Variety Care's policy to keep a record of all complaints and to investigate all complaints to determine the circumstances surrounding any concerns that patients raise
regarding the health center.
Procedure for Filing:
Patients may file a complaint in any one of the following ways:
a. in person, by discussing the complaint with the Variety Care contact person. The patient may be required to complete a complaint form. The contact person will assist with the completion of the form, if required.
b. by mail, by submitting a completed complaint form or in a letter containing the information specified to:
