Welcome Putnam North Family Medical Patients
Variety Care is excited to expand our mission of making quality health care affordable and accessible to the patients and community of Putnam North Family Medical Center!
We will begin remodeling the facility to enhance your experience. Remodeling will begin in early 2022 and we expect to complete the renovations in mid-2022.
During the renovations, our family medicine providers are prepared to offer you services at one of our metro health centers as well as telehealth options. For a smooth process, PNFMC will waive all fees associated with transferring medical records through December 31, 2021. After December 31, 2021, patients must pay a custodian fee to transfer their medical records.
To transfer your medical records from Putnam North Medical Center to Variety Care, you must request them from Morgan Records Management.
If you would like to receive medical care with Variety Care, at one of our metro locations, through telehealth or at Putnam after the renovations, please call 405-632-6688 and a Variety Care representative will assist you.
